The administration codes (90460/90461 for counselling-based admin, versus 90471/90472) are frequently mismatched to the patient's age and to how many components each vaccine has. Each missed component is revenue you already spent on stock.
VFC vs private stock accounting
Vaccines for Children doses have to be billed and inventoried separately from privately purchased stock. Mixing them up means either giving away paid inventory or billing for state-supplied doses — both are problems.
Well-child plus a sick complaint
When a preventive visit (99381–99395) also addresses an acute problem, the problem-oriented E/M is separately billable with modifier 25 — but only if the documentation supports both. It routinely gets dropped.
Screenings not captured
Developmental screening (96110), depression screening and similar are billable alongside the well visit and are often simply forgotten in the template.
